1 in 300: In your 30's your chances of getting pregnant after a tubal ligation is around 1 in 300. Though, given that you've had a pregnancy after a tubal ligation, i think your chances are higher. The tubes can recannulate (meaning then can attach back together). It's crazy, i know, but it happens. If you don't want to pregnant, i might use a back up method for birth control.

Be careful: Having a tubal ligation failure puts you at high risk for an ectopic pregnancy. Obviously, at least one tube may have re opened. So, it is possible that you might get pregnant. If you get pregnant, see your doctor right away.
