Do you as doctors/surgeons explain everything to the patient or if it was terminal would you leave some details out.....not to cause more stress?

Human conversations: A doctor-patient conversation is a human conversation, and like all human conversations, never is everything all said, and never is everything left out. Humans also never remember everything that was said, but only parts of conversations. Each doctor and each patient has his or her own style in a conversation. Each can ask the other for more details, but should avoid sounding too repetitive.
