Many things: If chronic and recurrent and made worse by sun exposure or alcohol or certain foods, it is likely rosacea. If acute or painful, it could be an infection called erysipelas which needs diagnosed and treated. The malar (facial) rash of lupus needs to considered. See your doctor or dermatologist. They can usually tell difference by looking. A picture is worth a thousand words!
