Get Right Diagnosis: Most hemorrhoids (varicose veins of anus) are soft, squishy tissue that stretch after straining/having a bm: no pain. If a hemorrhoid becomes firm, enlarged, and tender, it may thrombose (clot). This is treated without surgery w/pain meds and time. Any painful anal lump that is present all the time, however, requires medical attention to assure that the right diagnosis & treatment. Good luck!
