When?: A 'lump' is a scap hemorrhage qutside the brain and as such needs no particular treatment other than time, head injury can matter in old people. Those on anticoagulants and if it was associated with loss of consciousness or vomiting or iprolonged headache it should be investigated. Severe head trauma can be life threatening in the first 12 hours or so if accompany by a so called extradural hema.
