Usually lung: Could be bronchitis, bronchiectasis or copd, especially with a history of smoking. Could also be from silent sinusitis with postnasal drip. Or even gastric acid reflux. Your doc should be able to fix it.

Many things...: An infection, chronic bronchitis from smoking, asthma, severe allergies, cystic fibrosis, and many other things can cause you to have a lot of phlegm. If it doesn't go away, or gets worse, or gets bloody, see your doc.
