Practically anything: Any chronic illness would cause all of those symptoms. For example, advanced kidney disease, cirrhosis. Anemia, diabetes, thyroid deficiency, adrenal insufficiency to name a few. Any serious infection (for example sub-acute bacterial endocarditis) could also cause all of those sx.
