Deformed and painful: Most of the time, it comes out in the front of and below the level of the proper joint. The arm hangs down at your side and it is nearly impossible to raise it up or out. Trying to put your arm across your chest if extremely painful. There may be numbness in parts of the arm. It will not go back into place by itself, unless your clint eastwood in a movie. If it goes in by itself, it's not out.

Pain!: Immediate and severe pain and inability to use your shoulder. It is not subtle at all, but should be differentiated from a separated shoulder in which the Ac joint has been injured and is typically considerably less painful.
