Evaluation: An evaluation to include things like past psychiatric hx, family drug/alcohol/ psyche hx, subtance use, suicide & homicide risk assessment, current medications, allergies, herbals & supplements, family of origin hx, current sxs, sleep & appetite patterns. This is just a small sampling.
