Lactation : Your obstetrivian can help you with lactatation problems as can a mid wife. In this country we have lactation specialists who specifically trained in lactation problems.

Yes and no: Ob's & midwives alike are often finished with you after your hospital stay for the delivery and lactation support, if needed, is done by community groups and support nurses. But, some ob's, pedi's, family physicians, & midwives (esp homebirth midwives) do have a passion for topic & can be more supportive. A good question to ask prior to delivery is "who will support me if I have problems nursing?".
