Check adenoids: 2/3 of children have had an ear infection, also known as acute otitis media, before first birthday. Young children are susceptible to these infections in part because their eustachian tube not completely developed and horizontal instead of angled, easily clogging with fluid.Enlarged adenoids near the opening of the eustachian tubes, inflammation or enlargement of the adenoids may block the tube.

Further work up: He deserves further work up at this time. Chances are his ear infections didn't need antibiotic treatments as most of those are viral. Talk to your doc first and then you may need to be referred to specialist(s) if appropriate.
