Not a problem: The electrodes are not placed on the breast in anyone so it's not an issue. V1 goes to the right of the sternum; v2 (left 2nd ics) v4 in the mid-clavicular line 5th ics; v3 between v2 and v4. V5 in the anterior axiallary line 5th ICS and v6 in the mid-axillary line 5th ICS - none go on the breast which would act as an insulator and interfere with the tracing. Ics=intercostal space.

Same w/without impla: You essentially place them the same in patients with or without implants which tends to be towards the inside the right & left implants and then just below and to the outside of the left implant. Occasionally this requires placing the leads on the 5th intercostal space which does not typically interfere with the tracing.
