On the lung?: gosh could be quite a lot for this. If it is stable over years, cancer is MOSTLY ruled out (sometimes cancer can develop in a scar). And lung diseases such as COPD, or other causes of lung hyperinflation cause the bottom to get squished. For that matter obesity can squish them from the other direction!
