3 MOST COMMON!: 1. Inadequate intake: a true vegan will get no b12, except through supplements. 2. Atrophic autoimmune gastritis. The production of intrinsic factor is halted by immune destruction of parietal cells. 3. Inflammation of the terminal part of the small intestine, ie. Iliitis or crohn's, where B12 is absorbed. A new cause is chronic overuse of ppis, such as Nexium (esomeprazole) or prilosec.
