Depends: Some patients can have pain with normal us and hida scan. There are varying opinions among physicians as to the treatments. Options are- observation, diet modification, anti-spamsmodics (ie bentyl, (dicyclomine) levbid etc), anti-anxiety meds, surgery ( lap chole). U will have to discuss w ur pcp, GI doc and/or surgeon.

Sludge IS abnormal: It may be small, sand-like stones. To help determine if the gb is functioning poorly, i'd suggest not just the hida scan, but the hida scan should be performed with an ejection fraction being determined. The scan may show the gb fill (normal) but it may not empty well (evidence of obstruction is an indication to do surgery for the symptoms).

Need ejection: Fraction with hida, not just a regular hida scan. The hida scan looks at the production, storage and flow of bile. Adding the ejection fraction portion, using a hormone analog called kinevac, allows the determination of how well the gallbladder is emptying. Also, gallbladder sludge is an abnormal finding on ultrasound.
