See PCP first: Start with your primary care physician. However, if it is sudden and associated with any chest discomfort, shortness of breath, or nausea, seek immediate evaluate at an emergency room.

Specialist: For chronic (longer than three months) see an osteopath or physiatrist. They can provide a hand-on evaluation that is necessary to diagnose. Never have operation without second opinion. In my limited experience, shoilder operations are notorious for less than dramatic healing benefits.
