On the counter: Best thing to do is to rinse your toothbrush well after you use it and to place it upright on the counter, but not next to the toilet. This is better than putting it in the cabinet where it is a warm, wet and dark place where bacteria can easily grow. Remember to change you toothbrush every 3 months.
