Consult cardiologis: Cardiomegaly on radiograph is sometimes subjective. Heart size can change according to degree of inspiration and even hydration. Cardiomegaly can be caused by infection, congenital heart disease, cardiomyopathy, etc. If cardiologist hears no murmur, ekg normal, and cardiologist thinks physical exam is normal, tendency to ignore subjective impression on radiograph.

Consider echo : An x-ray is a faiy insensitive way to measure heart size. An echocardiogram, which is a simple non-invasive test, will give a precise measurement of cardiac dimensions. A little thickening of the heart muscle is normal in athletes.
