Depends on cause: Fat necrosis due to trauma to the fat usually heals by itself, though it is a slow process. Fat necrosis can also be part of systemic illness, e.g., diabetes and pancreatitis, in which case the underlying issue needs to be addressed. If it is traumatic fat necrosis, you may need to the address your or your partner's behavior.

No: A single patch of breast fat necrosis will leave a lump and a bit of deformity, which may be diagnosed on biopsy and/or removed surgically. It is unlikely to be due to any systemic disease, but dr. Singh is very wise to ask about injury.
