Yes and no: This is a hard question to answer given that pain perception are different for different people. Certainly a pap smear is not comfortable but should not really be painful.

No not ususally: It depends on who is doing it, how big your vagina is what size speculum is used what your pain threshold is , but it should only usually be not painful or only mildly uncomfortable, sometimes it is painful if you have a specific problems.

Can be: Pap smear can be slightly uncomfortable but are usually bearable . If someone has STI or infections like thrush then smears can become more painful .
