Multiple causes: Lumps under the skin in the scalp can have multiple causes including pilar cysts, sebaceous or epidermal inclusion cysts, lipomas, lymph nodes or other growth. You often cannot be certain what it is without taking a sample. Have the area evaluated by a plastic surgeon or dermatologist for removal and analysis by a dermatopathologist.

Hard to tell: Dandruff unrelated they may order a ct or mri or simply go to biopsy.
