Hyperthyroid Pre-op?: This depends on each individual, but some surgeons like to wait 6 weeks before drawing a tsh. Free T4 and T3 (liothyronine) should also be checked & used to calibrate your replacement therapy. Most surgeons place patients on a baseline amount of levothyroxine immediately post-op. A simple calculated regression equation taking into account age, weight (levothyroxine dose= bodyweight - age + 125) good predictor.

A long time!: Great question! after thyroidectomy levels may take 2-6 weeks to fall enough to notice. Since you listed Propranolol as a medication i'm guessing you had your thyroid removed because it was overactive. In that case your levels might be high for a while.

It varies but....: Thyroid hormone only last a few days at most in your bloodstream. If you have all your thyroid gland removed your levels will start falling within a day or two, but they may take a few weeks to level out with your medication dosage. There are two forms of thyroid hormone, each with its own activity level. There are two major ways these are metabolized and at least a dozen transport/receptors.

Starts immediately: The half-life of thyroid hormone is seven days, meaning that if no thyroid hormone enters your body, your current level will drop by one half in 7 days. If you're already on synthroid, (thyroxine) the dose may need to be adjusted slightly over the next several mos. Or may stay constant.
