Maybe GI pain: Addison's disease, or primary adrenal gland insufficiency, is a condition where the adrenal glands don't make enough cortisol, aldosterone, and adrenal androgens. These hormones control blood pressure, appetite, fluid balance, stress response, among others. The classic symptoms of severe addison's are extreme fatigue, dizziness, nausea/vomiting, pallor, salt craving, weight loss, skin darkening.

Not physically: Addison's is often diagnosed late, after months or years of simply not feeling well. Physical pain is likely to be late from muscle cramps from sodium wasting. But having no energy, swinging moods, nausea, weight loss, and being told you have mental health issues hurts in its own way.
