X-ray: An x-ray should determine if the sacrum has a fracture. Sometimes a ct scan may be needed if the x-ray is questionable.

Sacral insufficiency: Sacral fractures can also be subtle. CT must have thin cuts and reconstructed views. There are also types of fractures of a compressive (insufficiency) nature that are only seen on a special sequence of MRI called STIR (inversion recovery). If one cannot have an MRI, then a nuclear study may be warranted.
