Yes: Pituitary adenoma grows slowly. Most are small.

Possible, but: It is much more common after adrenal removal rather than failure. Usually the growth ls limited because the patient is on cortisone replacement or they would expire. This breaks the feedback loop and stops the pituitary stimulation. High fsh from ovarian failure happens in every postmenopausal female and is not a clinical problem.
