May be normal: If he seem well and exercises, this may be normal for him; many elite runners have heartbeats between 45 and 60. If the chest pain does not take his breath away, make him sweat, or otherwise suggest heart disease, it is probably chest wall pain; most athletic people experience some of this. If any doubt, he can visit his physician.

Slow heartbeat: Your boyfriend may be an athlete which would account for his slow heart rate. In addition, he may have a thin chest wall and he feels his heart beating up against his chest causing pain. Then again, he might have a heart block which could give him these symptoms. Therefore it is important for him to see his doctor for an evaluation and possible referral to a cardiologist.
