Clinical diagnosis : Unless angiogram performed which can confirm coronary spasm...Treatment can be similar with calcium channel blockers or nitrates...

Difficult: Some additional testing is needed to rule out coronary disease. Both may respond to Nitroglycerin e heart usually does not respond to antacids and GI meds. Age suggests non- coronary but other risks can prevail: smoking diabetes hyperlipidemia family history etc.
