Likely dryness: If you are within 0.75 d of plano (+/- 0.75), then the most likely cause of visual fluctuation is an unstable tear film. Try lubricating drops on a regular basis, maybe 3 or 4 times a day. You can also help to make the tear film more stable by taking omega 3 fatty acids and vitamin e (200 i.U.) as long as you have no medical contra-indications - check with your primary care doc, if in doubt.

12 months: It can take a full year to fully recover after lasik. You had a very high correction to start with. You are currently a bit undercorrected. There is a high chance that you will remain a bit undercorrected. You might be able to have an enhancement, but your cornea may be too thin at this point. You have had a reasonable outcome, given your high starting point.
