Yes easy: Yes general surgeon is trained to diagnose and treat them. Fissure is crack usually located near tail bone side ( 6 o clock position of anus ), fistula is tract with inter and external openings , some times more than one external opening.

Yes: Rectal fissure and fistula are two different problems. Both may result in bleeding or spotting but fissures are usually more painful and fistulas have an external opening (although you might not have noticed it). General surgeons deal with both of these, as do colo-rectal surgeons. They can both confirm and treat these conditions.
