Tear vs tunnel!: Anal fissure is a very painful tear of the sensitive anal lining, while a fistula is an infected tunnel that connects inside with outside. Often a fissure will heal without surgery, while a fistula usually won't, but get checked if you think you have either if these painful anorectal problems.
