Causes of hematuria : Cause of hematuria is multifactorial, it may not involve the prostate at all. Most common causes of hematuria are uti's, stones and tumors, coagulation disorders, trauma and glomerulonephritis.

Enlargement: Blood in the urine( hematuria) can have different sources. You asked about prostate, most common at your age will be the simple enlargement. The incidence of cancer also rises with age.

Here are some ...: Gross hematuria at 89, no matter what, should be thoroughly evaluated at least once with adequate imaging studies followed with cystoscopy. For imaging, US is not enough and CT.IVP is preferred if renal function is suitable. If not, US or CT without contrast can be supplemented with retrograde pyelogram - a special x-ray under fluoroscopy to see image of ureters and the hollow parts of kidneys.
