It's usually the doc: I'd guess some of us are "gentler" with the electrical stimulation initially to help the patient become accustomed and i personally talk quite a bit while doing the exam to distract the "victim" and try to avoid certain muscles which are more painful than others unless that muscle really needs to be examined.
