Why would nephrologist recommend catheter dialysis to my 90 yr old grandmother? demented, barely ambulatory and 16% function.

Dialysis for age 90: It seems that with a 16% of her renal function that your grandmother doesn't need to start dialysis (d). Most patients who are not diabetic start.

Depends: Some of her "dementia" may be caused by the kidney failure. A catheter is a quick, easy way to get her dialysis without too much risk. You can give her a few sessions and she if her mental state improves.

Catheter dialysis: I assume you are referring to peritoneal dialysis, using a catheter in her abdomen. This decision brings the family to a discussion of life goals. Extending her life with dialysis may not be the consensus decision. Hemodialysis is harsh for everyone, especially so for the frail elderly and was never intended for frail elderly. Peritoneal dialysis is gentle. At 16% function, she does not require D.
