Can early ultrasounds be off a few weeks?

Not really: Early ultrasounds are accurate plus or -7% loss if the embryo measures seven weeks it would be accurate to plus or - 3.5 days. If the ultrasound was done very early in the five week period when an embryo is not visible then they may not be quite as accurate. Repeat study in 1 to 2 weeks with ultrasound will then be definitive for dating your pregnancy. In general the earlier the ultrasound the more accurate the dating.

Usually 5 days : First trimester ultrasound is usually accurate to within 5 days. So if there is that much difference with the lmp, the LMP is suspect.
