Depends: Some variation in size is common as is the fact that one side may hang lower than the other (usually the left is lower). Varicocele and hydrocele may make one testis appear larger than the other and you may feel a lump above the testis. Only large hernias descend into the testis. Hernia and varicocele may result in a "dragging" type of pain but hydrocele is usually painless. Hope this helps.
