If it got: Worse after a shot to the point you had to go to the er, i would order an MRI to make sure you don't have a tear of the fascia or a stress fracture of the heel. Always, better safe than sorry.

Rest: A heel spur is actually bone formation at the origin of the plantar fascia (ligament that helps support your arch) a cortisone shot usually can relieve pain for 6 weeks, takes about 3 days to notice relief and you should not engage in sports for 2 weeks. Pain pills are not the answer. Rest for 2 weeks, replace any worn out non-supportive shoes and stretch your achilles tendon at least 2x/day.
