COPD?: Cop is criptogenic organizing pneumonia a rare cause of lung interstitial disease. I think you are talking about copd- chronic obstructive pulmonary disease. If he keep smoking the disease will continue to progress with increase incidence of pneumonia and respiratory failure. Also heart disease will progress with increase risk of heart attack.

Can't say but...: He might die tomorrow or live another 20 years- the issue is what will be the quality of life. With COPD he will feel more and more short of breath, develop chronic coughs and be able to do less and less activity. It's a terrible way to die. Stopping smoking is the best thing he can do.
