Communicate: Challenging, but can be done. First, with family, communicate your concerns/issue. I suspect you'll find they're willing to either reduce the salt they use, or at least provide some low-salt options. At restaurants, ask the waiter which options are lower in salt. If they don't know they can ask the chef. Dine at places that are receptive. Talk with a dietitian about other strategies that can help.

Be diligent!: It won't be easy, but if you are diligent with your research of a restaurant's menu online before you go out to eat you can often find detailed nutrition information. Additionally, you can call ahead and speak to the head chef specifically about your dietary restrictions/requirements to make sure they can have appropriate food available. With family, you just have to explain and be careful!

Low salt options?: There's alot of national attention now towards salt content of food. The restaurant industry is getting a lot of pressure to disclose the salt content in their food, and offer healthier alternatives. Hopefully the place you go to has low salt options. If not, limit the quantity of food you eat. Eat less of the "sauce" in food. Alot of salt hides in the sauce.Good luck.
