Likely the same one,: Whatever is causing this uti/sepsis is likely the same one. It was probably treated with either the wrong antibiotic(s) or not enough of the right one. Getting a blood and/or urine culture or gram stain of the urine would be very helpful in directing antibiotic therapy, plus sepsis should be treated with IV antibiotics, as there is a need for a greater concentration in that case.
