See a doctor: It sounds like you are having significant problems. You need a thorough evaluation to help sort out what is causing them. I am sorry things are causing you this much trouble now and i wish you the best. Sorry i can't be more specific here, but there are many possible contributors and a good doctor will sit with you and help sort out what's going on.
