Not really "fail": It's not really "failing" the stress test -- but if you have ekg changes while on the treadmill the doctor will most likely want you to stop. Or, you could develop chest pain or other symptoms while exercising. Your doctor will review the results with you and make recommendations for further testing if needed, and treatment.

Target heart rate...: If you are having an exercise stress test, there is a target heart rate which needs to be reached. This target heart rate is 85% of your maximum predicted heart rate. Maximum predicted heart rate is given by equation (220 - age). If you do not reach target heart rate, the exercise test may need to be converted to a pharmacologic stress test. Therefore you have "failed" to exercise adequately.
