It your ENT doctor does not think this is an ear issue, then a neurologist should examine you next to decide on the diagnosis and treatment. You may need additional testing as well but the neurologist will be able to determine what additional tests, if any, are needed. A hearing loss could give the tinnitus so a hearing test should have been done, hopefully by ENT. So, see a neurologist for eval.

In cases like yours I recommend consulting a specialist in Neuro Otology in a department of otolaryngology at a university hospital near you. Bring all pertininent reports to avoid duplicate testing.
