Off and on extreme fatigue for several years. i tested positive for gastric parietal cell antibodies. negative for intrinsic factor. b12 is 274. should i be worried about pernicious anemia? should i see a hematologist or gastroenterologist?

You may very well have pernicious anemia but regardless of etiology, your B-12 deficiency still needs treatment, and would suggest monthly injections. A hematologist would be an appropriate specialist, but most primary care physicians should be able to handle your needs.
