The pharmacist is correct that novorapid dose should be adjusted by fasting blood sugar before each meal. However, if you are also taking long acting insulin (basal insulin) then doctor may decide you just need only a fixed dose of rapid acting insulin before each meal for preventing too high postprandial glycemia without the need of knowing your glycemia before meal and the dose is usually low.
