Generally there will be an injection of numbing medicine into the nerves at the root of the tooth, and then with various tools the tooth will be removed painlessly. The area might be packed with gauze or sometimes an absorbable suture may be put in to prevent bleeding and improve healing.

Depends somewhat on the complexity of the procedure. Surgical removal (for example for an impacted tooth) is more complex than straightforward extraction. In either case you will receive appropriate anesthesia. Topical medication and/or suture(s) may be required. Some post-surgical discomfort controllable w medication.
