The blood supply to the testicle has to be cut off for atrophy & shrinkage to gradually develop. Instead of worrying about what is unlikely in your case, you should be examined & advised by your physician or a urologist. Any appropriate testing will be ordered. You may have epididymitis which is treated with antibiotics.

Your symptoms warrants a visit to a Urologist- now -if that is not possible go to the ER and get checked out. It takes several months for a 'dead ' testicle to shrink. Good luck
