Less than 5% when done by a root canal specialist, an Endodontist.

The most common cause for failures are complex branching of nerves such as a horizontal branch, and multiple canals that can't be accessed or even seen. At least 95% or more root canals are successful. Sometimes a root can be fractured from decay or trauma and an extraction is the only option.
