The basic answer is that the immune system reacts to some stimulus in a abnormal (auto-immune) way. An example is when the body has a strep throat a portion of the population will develop antibodies to strep (which is normal) but those antibodies attack the heart mitral valve or heart muscle causing damage (the abnormal or auto-immune part). That condition is known as Rheumatic fever,
