Big toe paronychia 2 weeks.topical & 10 days oral antibiotics failed. (& 2x daily epsom 15 min soaks.) dpm: no pus visible to drain.symptoms:lateral focal tenderness,blanching, upper toe pinkish.but, small improvement on day 10 of orals. now what?

You are correct. It should have healed with that regimen. The doctor should probe and look at the area for a nail spicule There is probably a sharp piece of nail, still irritating, the tissue, not allowing it to heal. if there is not a nail spicule portion of the nails should still be removed.
