Most likely he has a bacterial infection which should resolve in the next week or so. If the blisters worsen,spread or don't improve, consider MRSA and a culture. Sounds like impetigo (staph infection) Hot compresses, Clindamyciin, Bacroban ointment. Wrap the childs hand up so they can rub on the ears and then elsewhere which cause spread. I never seen a Case of Ramsey Hunt in a young child (unlikely) is child immune compromised.
